Submit a Claim - Claim Instructions

If you are a member of the Settlement Class and wish to receive an award under the Settlement, you must submit a valid Claim Form so that it is postmarked (if mailed) or submitted online no later than May 22, 2025.


How to File Online:

A Unique ID is required for login and submission. Please see the Notice sent to you for your Unique ID. If you do not have your Unique ID, please contact the Class Action Administrator at 1-888-884-7716. If you have lost your Postcard Notice, you may also log in by entering the phone number at which you received text messages from Citizens Disability, LLC.

How to File by Mail:

Fill out, sign, and date the Claim Form, and then mail the Claim Form to the following address:

Remember, all Claim Forms must be submitted online or by mail. If submitted by mail, the Claim Form must be postmarked or received by the Class Action Administrator no later than May 22, 2025.
